Administration of Medicines
Where possible student medication should be administered by the student or be administered by the parent/guardian at home in times other than school hours. As this is not possible in all instances, the Principal approves that school staff administer prescribed medication to students, as long as the following requirements are being met.
Anaphylaxis Action Plan
If a student is allergic and has Anaphylaxis reactions, please ensure we have a ASCIA Anaphylaxis Action Plan, signed and dated by your Medical Practitioner.
Drug (Medication) Allergy
If a student is allergic to any form of medication, please ensure we have a ASCIA Drug Allergy Action Plan, signed and dated by your Medical Practitioner.
Asthma Action Allergy
If a student has asthma, please ensure we have a NAC Asthma Action Plan, signed and dated by your Medical Practitioner.
Parish Priest Reference
The Catholic Education Commission of W.A. Policy Statement on Student Enrolment requires the enrolling Principal to consult the parish priest.
Completion of this form and presentation to the parish priest forms part of the enrolment process for Matthew Gibney Catholic Primary School. Contact should be made with the parish secretary to find out the process for that parish.
